SCHEDULING AND GETTING STARTED
1. Schedule an initial telehealth appointment online. For weight loss appointments, which include medication, payment is requested at the time of scheduling. For primary care appointments, payment is requested at the time of the visit.
2. Complete the online patient forms prior to your visit so our clinicians have time to review them. If your forms have not been completed by the night before your appointment, it may be rescheduled.
3. After a weight loss appointment, a prescription for one month of medication will be sent to the compounding pharmacy. The pharmacy will contact you regarding delivery date and provide you with instructions. Compounded GLP-1 medication expires after 30 days.
OBTAINING A GLP-1 REFILL
1. Followup appointments are required monthly for the 1st 3 months. After 3 months, quarterly appointments are required at 6, 9 and 12 months.
2. In between quarterly visits, you have the option of scheduling a regular monthly refill visit with your provider, or you can request a refill via the portal if you would like to skip the in between visits.
3. Make sure to allow a week’s notice for refills in case the pharmacy is testing a new batch.
Do you qualify to be prescribed GLP-1 medication?
GLP-1’s can be prescribed to people with obesity, or people who are overweight with a BMI > 27 that have a health condition related to their extra weight. If you feel you are overweight but your BMI is less than 27, please submit a photo. BMI is the ratio of your body surface area to your height and is calculated using height and weight. It does not take into account size of skeletal structure or fat vs. muscle mass and so the number is a general guide.
Long Term Use
**PLEASE NOTE: If you are on this medication long-term, a copy of your annual physical and annual lab work by your PCP will need to be uploaded to your bellobella health record every 12 months
Mature Adults
If you are 65 or older, proof of a physical and lab work within the last 12 months is required prior to your first prescription.
Alternatives for non-GLP-1 Candidates
**IF YOU HAVE DIABETES MELLITUS TYPE I, OR A HISTORY OF MEDULLARY THYROID CANCER OR MULTIPLE NEOPLASIA SYNDROME TYPE 2 THEN YOU ARE NOT A CANDIDATE FOR GLP-1 AGONISTS. We may be able to provide you with an alternative weight loss medication after a complete evaluation.
